Understanding the situation
A complete assessment at home: health, independence, medication, nutrition, surroundings and support network.
Nursing care and follow-up at home
Nursing assessment and follow-up to understand the real situation, organise care and give the whole family peace of mind.
A nurse’s perspective
Many important things do not fit into a medical appointment: how someone is eating, whether they are walking as safely as before, if they understand their medication, or whether a small change has begun. Paying attention to all of this is my job.
When you want to know how they really are
Sometimes a phone call is not enough to understand the real situation. A professional visit makes it possible to calmly observe how the person is doing, what has changed and whether support is needed.
How I support you
A complete assessment at home: health, independence, medication, nutrition, surroundings and support network.
Regular visits to recognise changes early and act before they become a problem.
Clear recommendations and help organising care, making decisions and preparing for specific moments, such as a trip, celebration or change of routine.
After each visit, the family receives a clear explanation and a report.

Always the same professional
I will carry out the assessment, visits and follow-up myself. You will not have to explain everything from the beginning: I will know the person, their home, their routines and what concerns the family.

Understanding medication, learning to use a device and knowing what to look out for.

Recommendations tailored to the person, their home and their routines.
Could this help your family?
—Your mother or father lives alone and you want to know how they are really doing.
—There has been a hospital stay or a change in health and you need reassurance when they return home.
—Medication, falls, loss of independence or small changes are causing concern.
—There is a caregiver and you want the care to be well guided and supervised.
—A newborn baby, a younger person or someone with a chronic condition needs care and guidance at home.

Supporting the family too
I help you understand what is happening, weigh up options and make realistic decisions. With your permission, I can be the point of contact with the doctor and other professionals, bring information together and support continuity.
Who will be by your side
I have been a nurse since 2007. Emergency care taught me to observe with rigour; nursing management taught me to organise care; and throughout it all, I have never lost sight of the person behind each situation.
My training in nutrition, dietetics and healthcare leadership helps make care at home clearer, safer and more manageable.
